About the role

EzyHelpers is a trusted home care and domestic helper service provider based in Bangalore. We help families find reliable, trained, and verified caregivers, nursing assistants, home helpers, cooks, nannies, elderly care attendants, and patient care support staff. As we expand our healthcare and caregiving services, we are looking for a dedicated and responsible Care Coordinator – Home Healthcare who understands the caregiving ecosystem and can coordinate effectively with nurses, physiotherapists, caregivers, patients, and families.

This is an office-based coordination and operations role. You will understand patient and family requirements, coordinate with staff nurses, nursing assistants, physiotherapists, and caregivers, manage caregiver availability, and ensure smooth, timely service delivery and follow-up for clients. The ideal candidate has experience across elderly care, patient care, post-operative care, palliative care, physiotherapy support coordination, and home-based caregiving services.

What you'll do

Your day-to-day responsibilities

  • Speak with families to understand care requirements in detail — elderly, post-operative, palliative, bedridden, dementia, physiotherapy support, general caregiving, or nursing support.
  • Ask the right questions to assess the patient’s condition, mobility level, medical dependency, food habits, language preference, duty timings, and family expectations; document everything in the internal tracker.
  • Coordinate with staff nurses, nursing assistants, GDA/ANM/GNM candidates, physiotherapists, and caregivers; check availability, experience, location, language skills, and suitability.
  • Match the right caregiver or healthcare support staff to client needs, and brief them on patient condition, family expectations, hygiene standards, duty timings, and responsibilities before assignment.
  • Manage day-to-day coordination between clients, caregivers, nurses, physiotherapists, and internal teams; follow up after placement to ensure satisfaction.
  • Resolve issues around caregiver behaviour, attendance, duty understanding, replacements, or service gaps; arrange replacements and ensure continuity of care without disruption.
  • Help families choose the right level of care based on the patient’s condition, and escalate medical or clinical concerns to qualified professionals when needed.
  • Screen and profile caregivers, nursing assistants, staff nurses, and physiotherapists by skills, past cases, language, location, salary expectation, shift preference, and availability; maintain an updated staff database.
  • Maintain polite, clear, professional communication with families; give realistic updates without overpromising and handle concerns with empathy and ownership.
  • Coordinate with the sales and operations team for closures and service delivery.
  • Maintain accurate records of requirements, staff profiles, placements, follow-ups, complaints, replacements, and service status; prepare daily/weekly reports on active cases, pending requirements, staff availability, and escalations.

  • Strong communication in English and Hindi; Kannada is an added advantage.
  • Ability to coordinate with caregivers, nurses, physiotherapists, and families.
  • Strong follow-up and documentation skills.
  • Empathy and patience while handling families and elderly care cases, including sensitive healthcare conversations.
  • Good judgment in matching the right caregiver to the right family.
  • Comfortable working from the office 6 days a week.
